If you do not want to use the Online Ordering, you may order by phone, fax, e-mail or mail. You may fill out, print, and fax or mail the Offline Order Form.

Prices include USPS First Class/UPS shipping and handling charges. For Federal Express, add $15 for the first item and $15 for each additional one. Please provide your street address.

Prepayment is required for all International orders with the exception of Government Agencies and Universities.
